Maryland Independent Living and Home Health Care

The objective of this program is to coordinate a plan of care when you leave the hospital.

Topics On This Page

This program provides Home Health Help from the Baltimore County Department of Health

The service can provide care to seriously ill patients. The goal is to prevent costly nursing home or hospital admission.

What kind of help is available?

In-home health services can be for intermittent skilled nursing, physical, occupational and speech therapy, medical social service, and home health aides. Some medical supplies might be covered, and medical equipment like walkers and wheelchairs might qualify for the program. Licensed nurses provide home health care. Home health aides provide assistance with daily living, such as bathing, using the toilet, and dressing.

You must be a resident of Baltimore County and a doctor's order is required. Your physician must decide you need home health care and order a plan of care. (A plan of describes what kind of care you need, what type of health care professional is required, and how often you need the services.)

You must require part-time skilled nursing, and you must be home-bound.

The cost is covered by Medicare, Medicaid, your private insurance, or your private payment. Inability to pay does not mean you won't receive service.
